Update your Manitoba Health Card

Name Change

If you legally change your name, you are required to report the change to Manitoba Health to remain eligible for coverage.

To report a name change, follow these three steps:

Gather one of the following documents

Marriage Certificate (with an English/French translation)

A document issued by a governmental branch, which confirms a marriage has taken place. It may be used to assume a married surname on a Manitoba Health card.

Divorce certificate (with an English/French translation)

A document issued by a provincial/territorial court (or other jurisdiction) which confirms that a divorce has been finalized and the parties named are free to re-marry. It is used as proof that a person may legally re-assume a maiden surname if they were previously using a different surname during the marriage.

Legal Name change document

A document issued by a provincial/territorial Department of Vital Statistic branch. This document can be used to change a person’s name on a Manitoba Health card.

Updated Immigration document

An updated document (Canadian Citizenship certificate/Permanent Residence Document/Permit) from Immigration, Refugees and Citizenship Canada reflecting a change has been made to the person’s name on their Immigration documents.

Complete the form

Before starting the online form:

Changes can only be made by the card holder, their Power of Attorney/Substitute Decision Maker/Committeeship holder.

Power of Attorney, Substitute Decision Makers and Committeeship holder documents must be on file with our office, if not please contact 204-786-7101 or 1-800-392-1207(toll-free) and we will provide information on the document process.

All forms must be signed in ink or with a verified digital signature prior to being submitted to Manitoba Health.

You will need your Health Card Registration Number and Personal Health Identification Number (PHIN). You can see where these numbers are located on your Manitoba Health Card here.

If you do not have your health card information available, please contact 204-786-7101 or 1-800-392-1207 (toll free).

To submit your request, print sign and send the completed form and copies of the required documents to one of the following:
