Change in Immigration Status

If you have received Permanent Resident status in Canada, you are required to report this change to Manitoba Health, Seniors and Long-Term Care to remain eligible for coverage.

To change your immigration status, follow these steps:

1. Gather the required documents

You will need to provide an additional document along with the completed form.

You must submit a copy of one of the following documents:

Confirmation of Permanent Residence (landing document)

A Confirmation of Permanent Residence (COPR) document is issued by the Canadian government, confirming your permanent resident status and the date permanent resident status was conferred.

Permanent Resident Confirmation card (copy of both sides)

A laminated identification card that is issued by Immigration, Refugees, and Citizenship Canada confirming permanent resident status.

2. Complete the form

Before starting the online form:

You will not be sent a new Health Card for updating your Immigration Status.
If you would like a new Health Card, there will be an opportunity to request one once you submit this request.

The form can only be filled out by the cardholder.

You will need your Health Card Registration Number and Personal Health Identification Number (PHIN). You can see where these numbers are located on your Manitoba Health Card here.
