Address Change
You are required to report any change of address to Manitoba Health, Seniors and Long-Term Care to remain eligible for coverage.
Before starting the online form:
You will not be sent a new Health Card for updating your address.
If you would like a new Health Card, please use this form instead: Replace your Health Card.
The form can only be filled out by the cardholder.
You will need the following information:
Your Health Card Registration Number and Personal Health Identification Number (PHIN).
You can see where these numbers are located on your Manitoba Health card here.
