Add a spouse
To add a spouse to your health card, follow these steps:
1.
Gather the required documents
One of the following documents must accompany your completed form:
Copy of Mariage Certificate
Please include a French or English translation
Signed common law declaration form
Common Law PDF FormIf your spouse is arriving from outside of Manitoba, their date of arrival in Manitoba must be indicated on the form, and you must provide copies of documents to show proof of their legal status in Canada.

Complete the form
Before starting the online form:
You will be sent a new Health Card once your request is approved.
Changes can only be made by the card holder, their Power of Attorney/Substitute Decision Maker/Committeeship holder.
Power of Attorney, Substitute Decision Makers and Committeeship holder documents must be on file with our office, if not please contact 204-786-7101 or 1-800-392-1207(toll-free) and we will provide information on the document process.
All forms must be signed in ink or with a verified digital signature prior to being submitted to Manitoba Health, Seniors and Long-Term Care.
You will need your Health Card Registration Number and Personal Health Identification Number (PHIN). You can see where these numbers are located on your Manitoba Health Card here.
