Add a dependant
To add a dependant, follow these steps:
1.
Gather the required documents
You will need to provide additional documents along with the completed form.
You must submit a copy of one of the following documents for the dependant that is being added:
An official birth certificate issued by the vital statistics agency of a Canadian province or territory. These documents are not eligible: divisional or district birth certificates; commemorative birth certificates, Baptismal certificates, and notices of live birth issued in Manitoba.
A plastic card issued prior to Feb. 1, 2012.
A letter-sized certificate issued after Jan. 31, 2012.
This card confirms status as a Treaty Indian under the Indian Act.
Your visitor record is issued by the Government of Canada and satisfies requirements for date of birth, legal name and entitlement to be in Canada. If accompanied by a foreign passport, it also satisfies requirement for photo.
2.
Complete the form
Before starting the online form:
You will be sent a new Health Card once your request is approved.
The form can only be filled out by the cardholder.
You will need your Health Card Registration Number and Personal Health Identification Number (PHIN).
